Additional Visa Pages

You need to use DS-4085 in order to get additional pages in your passaport. This form should only be used to add visa pages to previously issued, currently valid U.S. passaports.

Please follow the instructions on the form carefully. Complete, sign, and date the application. If you want to apply by mail, please include your passport, completed form in envelope and mail them to appropriate office below:

If you reside in Afyon, Agri, Aksaray, Amasya, Ankara, Antalya, Ardahan, Artvin, Aydin, Bartin, Bayburt, Burdur, Cankiri, Corum, Denizli, Erzincan, Erzurum, Eskisehir, Giresun, Gumushane, Igdir, Isparta, Izmir, Karabuk, Karaman, Kars, Kastamonu, Kayseri, Kirikkale, Kirsehir, Konya, Kutahya, Manisa, Mugla, Nevsehir, Nigde, Ordu, Rize, Samsun, Sinop, Sivas, Tokat, Trabzon, Usak, Yozgat and Zonguldak, please send your application to the Embassy in ANKARA:

Your passport will be returned to you by UPS within 5 working days and UPS will charge $5 or equivalent Turkish Lira from you at the delivery.

Method of Application:

You can mail in your application for additional pages by following the procedure above or you can apply in person in Ankara. If you want to apply in person in Ankara, an appointment is necessary. To book an appointment, please follow this link.
