Electronic Visa application Form (EVAF)

 

  • The visa application form DS-156 must be filled out on-line. Complete this form ahead of time and bring a printed, signed and dated copy to your interview.
  • You will not be admitted for your visa interview without a proper, electronically-completed DS-156 form with the 2D barcode on page 3.  Hand-filled or incomplete DS-156 forms cannot be accepted.
  • The information that you submit is not stored nor is it submitted electronically to the visa unit. It is only used to generate the 2D barcode on page 3, which contains the data you enter.

What you Need

Please have your passport, previous visa information (if applicable), and any other documents relevant to your intended travel to the United States ready and available for your reference before you start the application process.

To use the electronic visa application form, your Internet browser must support 128-bit encryption and you should use a laser printer to print the completed application form. If you are using Internet Explorer (Windows), the minimum version that will work with this site is version 5.0, with service pack 2. If you are using Netscape, the minimum version that will work with this site is version 6.2. You must also have Adobe Acrobat Reader installed on your PC in order to download and print the completed application form. The minimum version that will work with this site is version 5. If you do not have Adobe Acrobat, you may download a copy from Adobe’s web site.

Depending on the speed of your Internet connection, it may take several minutes to download the initial and completed copies of the form. Please be patient.

 

Filling in the electronic DS-156

  • Complete the form on-line by filling in all applicable data fields. Fill in the form as completely as possible leaving blank only those fields that do not apply in your case. If a field is completed incorrectly, a message will advise you on how to correct the entry.
  • You must use English characters when entering your information. Letters like ü, ı, ç, and ğ are not recognized by the system. Please enter names like Gönen and Ertuğrul as Gonen and Ertugrul, unless otherwise asked.
  • Do not enter "N/A" or "YOK" in any field.  If a question does not apply to you, simply leave it blank.

  • When entering Turkish passport numbers (question 1), enter the full number including three letters starting with TR, then six numbers, with no spaces or dashes between then, like this: TRX123456
  • Women who have adopted a different surname after marriage or divorce must provide their maiden name and any other names previously used (both first name and surname) in questions 8 and 9.
  • Enter Turkish phone numbers in the following format: 90-212-333-3333, and U.S. phone numbers as follows: 1-202-333-3333.
  • If another person prepares the DS-156 form on behalf of the applicant, they should check "yes" to question 39, provide the information requested in question 40, including their signature.  (The applicant himself/herself must always read and sign question 41.)

Completing and printing the electronic DS-156

  • After answering all questions, carefully review the information you have entered for accuracy.  Inaccurate data in your DS-156 can lead to problematic errors on your visa.
  • Press the "Continue" button at the end of the form and wait while the system processes your data and transfers it to a three-page DS-156 form with a special 2D barcode on page 3.  Failure to click on the Continue button will result in an incorrectly formatted application form.
  • Print the three-page form after it appears on the screen.  The applicant must read and sign/date question 41.  Bring all three pages, including page 3 with the 2D barcode, to your interview.
