First Time Passport Applicants and Renewals for Certain Conditions

Please follow the instructions on this page if:

  • You have never had a passport issued in your own name;
  • You are under the age of 16;
  • Your most recent passport was issued before you were 16 years of age;
  • Your most recent passport was issued more than 15 years ago; or
  • You have been advised by this office to apply in person for the passport.

Note: If you are applying for the first passport for a child under the age of 18 who was born overseas, you must first report the child's birth.

Application Requirements

It takes approximately two weeks to complete a passport application (for us to send a request for a passport to the United States, receive the new passport and mail it to you). Please apply early for your new passport. To apply, please bring the following documents to the nearest Embassy or Consulate:

  • Two identical passport photographs: The Embassy/Consulate will reject any photographs that do not meet the specified requirements
  • Fees: The application fee for an applicant under the age of 16 is U.S. $85.00; aged 16 and over is $100.00. You may pay by cash or credit card at the Embassy/Consulate.
  • Your most recent United States passport (if applicable) and a photocopy.
  • Your original birth certificate or Consular Report of Birth Abroad (if you are a first-time applicant OR if you are under the age of 16) and a photocopy.
  • Up to date form of identification and a photocopy. This may be in the form of:
    • a valid passport of any country
    • a valid driver's license with photograph
    • a national identity card with photograph
    • a valid U.S. or State government ID card or
    • a U.S. military ID card.
  • Proof of eligibility for citizenship: A copy of the US passport of one of your parents or a copy of your mother’s visa indicating her status in the US when you were born.
  • Name change documentation: If you have changed your name since your last passport was issued, you will be required to furnish name-change documentation, such as your marriage certificate or other legal document.
  • If applying for a child under the age of 16, you will also be required to follow these additional instructions:

All children under the age of 16 are required to appear at the Embassy/Consulate with both parents for all passport services. The only exceptions made are in cases where one parent is outside of Turkey or is physically unable to travel to the Embassy/Consulate (for example, they are seriously ill).

Parents accompanying their child to the Embassy/Consulate must bring their valid U.S. or foreign passports one photocopy of each.

If only one parent is able to appear, one of the following will be required in addition to the above:

  • Form DS-3053 (Statement of Consent) completed by the absent parent must be notarized before a U.S. or Turkish Notary Public. American Embassies and Consulates abroad can also notarize Form DS-3053 (Statement of Consent).
  • Copy of the absent parent’s U.S. or foreign passport OR
  • Evidence of the appearing parent's sole authority to apply for the passport in the form of one of the following:
    • Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or
    • Child's birth certificate listing only applying parent; or
    • Court order granting sole custody to the applying parent (unless child's travel is restricted by that order); or
    • Adoption decree (if applying parent is sole adopting parent); or
    • Court order granting sole custody to the applying parent (i.e. the absent parent has no access to the child. Child's travel must not be restricted by that order); or
    • Judicial declaration of incompetence of non-applying parent; or
    • Death certificate of non-applying parent.
