Report Birth Abroad

Replacement Certificates

How to Apply for a Replacement or a Certified Copy of Consular Reports of Birth (FS-240 & DS-1350), Death (OF-180), Certificate of Witness, Marriage Records, and Panama Canal Zone birth and death certificates:

Submit a signed, written request including all pertinent facts of the occasion along with requester's return address and telephone number. Only the subject, parent, or legal guardian may request a birth record. If a birth record is to be amended or corrected, appropriate certified documents supporting the request must be included 

  1. The request must be notarized by an embassy or consulate 
  2. The request must include a copy of a valid photo identification of the requester (driver's license, passport, etc., even if the requester is not an American citizen).

These requirements apply to requests for documents from the Vital Records Office of Passport Services in Washington, and not apply to first time applicants requesting services from an Embassy or Consulate.
