STEP 4: Complete the Application Forms

All non-immigrant visa applicants must complete the Electronic Visa Application Form (DS-156).  If this form is not completed, the appointment may be cancelled and a new appointment will be required.

Click on http://evisaforms.state.gov to complete the Electronic Visa Application Form.

Advice on completing the Application Form

  • Applicants must answer every question on all forms. Please ensure all answers are written clearly. If a question does not apply, please mark it “N/A.”  Enter the information requested into the appropriate spaces on the form. 
  • Your answers must be in English and must use English characters.  Letters like ç, ğ, ı, ö, ş and ü are not recognized by the system.  Please enter names like Ayşe and Ömer as Ayse and Omer.
  • Cell phone numbers and telephone numbers should be entered as country code – area code – telephone number.  (i.e., 90-312-455 5555 for Turkey and 1-212-555-5555 for the U.S.)
  • Press the “continue” button at the end of the form.  Failure to click on the “continue” button will result in an incorrectly formatted application form.
  • Print all three pages, including the last page, which contains your barcoded information.  The barcode that prints on the last page contains the information you entered and will help us process your application more quickly.  If the barcode on the third page of the DS-156 form is faulty (i.e. the lines on each side of the barcode are not straight), fill in another DS-156 form online and print it out.  (Filling in more than one form will not create a problem.)
  • If the applicant receives assistance from another person with any part of the application form, that person must complete question 40 and sign the application.
  • Make sure you sign the form on item 41.
  • Bring all printed pages with you when you come in for your interview.  Please do not fold the printed application

Turkish male applicants between the ages of 16 and 45, and all male and female third-country applicants, are required to complete the Supplemental Non-immigrant Visa Application Form (DS-157).  An electronic version of a DS-157 form is not available.  Please use this link for the DS-157 in English.

The DS-157 is not required for Diplomatic (A1 and A2), International Organization (G1, G2, G3, G4), and NATO visa applicants.

Applicants traveling to the U.S. with an I-20/M-20 (students) or a DS-2019 (exchange visitors) must complete the Contact Information and Work History for Non-immigrant Visa form (DS-158) in addition to the DS-156 and DS-157.
